2. Begin by entering your Tax Year and LA Revenue Account Number or SSN at the top of the form.
3. In Column 1, list the states where you earned income. Ensure accuracy as this will affect your tax credit.
4. For Column 2, input the income earned in each state that is taxable to Louisiana.
5. In Column 3, enter the net tax liability reported on your other state's return.
6. Calculate and fill out Column 4 by determining the percentage of income earned in that state relative to your total Louisiana adjusted gross income.
7. For Column 5, multiply the percentage from Column 4 by your Louisiana income tax amount.
8. Finally, in Column 6, enter the lesser amount between Columns 3 and 5 for each line. Sum up Lines 1 through 19 for Line 20.
