1. Click ‘Get Form’ to open the military training request in the editor.
2. In SECTION 1, STUDENT DATA, fill in your NAME, RANK/GRADE, UNIT, and SSN. Ensure you provide accurate information for SECURITY CLEARANCE and DATE OF RANK.
3. Complete the remaining fields in SECTION 1 including PHONE, GENDER, CIVILIAN JOB SERIES/PAY PLAN, and PROM PTS. If applicable, attach a copy of your PROFILE.
4. Move to SECTION 2 for COURSE INFORMATION. Enter the COURSE TITLE and COURSE NUMBER along with the SCHOOL name. Provide a PURPOSE/JUSTIFICATION for the training.
5. In SECTION 3, COMMAND APPROVAL FOR REQUEST, ensure that each commander’s approval is documented by selecting CONCUR or NONCONCUR and adding dates.
6. Finally, in SECTION 4, STATUS OF TRAINING REQUEST, indicate whether you have been confirmed for course dates or placed on a waiting list.
