5. Indicate your Highly Qualified status by checking 'Yes' or 'No' for each certification area listed. Ensure you also provide Certification Area(s) and Grade Levels.
6. Check your Contract Status and indicate if you have been identified as a Unit Adjustment Transfer (UAT)/Excess employee.
7. Finally, submit the completed form to your principal or director within two business days of applying online. You can send it via fax, email, or hand delivery.
