Birth certificates are available approximately 10 days after the child is born. Birth certificates for Utah births that have occurred anywhere in the state from 1905 to the present may be obtained here.
What identification is required to obtain a birth certificate? A current, government-issued photo ID is required to request a birth certificate.
In person orders can be placed at most Utah Local Health Departments. All requests require a complete application, correct fees and valid identification. Local Health Departments provide copies of applications if you are unable to print one.
Birth, stillbirth, and death certificates can be requested in person at any of the office locations. A current photo ID and a valid form of payment are required. If requesting a certificate for a relative, proof of relationship, such as a birth or marriage certificate, may also be needed.
A biological father can be added by a Voluntary Declaration of Paternity if all parties are willing. The mother must start the request. To begin the process submit a completed Fathers Worksheet, a Birth Certificate Application completed by the mother, a copy of the mothers ID, and the correct fees.
