Certificates of Immunization (Form 3231) can be downloaded from the Georgia Registry of Immunization Transactions and Services website at . To gain access to GRITS and enrollment information email the GRITS Help Desk at dph-immreg@dph.ga.gov.
Exemptions must be updated annually by a qualified physician. Reasons for medical exemptions can include previous adverse reactions to vaccine ingredients, autoimmune disease, or established immunity from a vaccine preventable disease through titer testing. Exemptions must be signed off on using Form 3231.
State of Missouri Those vaccinated in Missouri can complete an online form to receive a copy of their immunization records from the states registry, ShowMeVax. Completed forms can be sent by email or fax to 573-526-0238.
