If youre not sure whether a case is recordable, call your local OSHA office for help. Be sure to transfer these totals to the Summary page (Form 300A) before you post it.
EMPLOYERS REPORT OF OCCUPATIONAL INJURY OR ILLNESS.
You must complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log.
You must record any docHub work- related injury or illness that is diagnosed by a physician or other licensed health care professional. You must record any work- related case involving cancer, chronic irreversible disease, a fractured or cracked bone, or a punctured eardrum. See CCR title 8, 14300.7.
Establishments that meet certain size and industry criteria are required to electronically submit injury and illness data from their OSHA Form 300A, 300, and 301 (or equivalent forms) once per year to OSHA. OSHA collects this work-related injury and illness data through the Injury Tracking Application (ITA).
