In Part II, indicate the type of update you are making by ticking the appropriate box for New/Additional Employment or Termination of Employment. Complete Parts III and V for new employment or Parts IV and V for termination.
For Part III, provide details about the new or additional employment, including the date of commencement, company name, Unique Entity Number (UEN), address, contact information, occupation, job description, and salary details.
In Part IV, if applicable, enter the date of termination and company details. Ensure that a representative's declaration is signed in both Parts III and IV.
Finally, complete Part V with your declaration as a PEP holder and sign it before submitting via email to MOM_PEP_notification@mom.gov.sg.
