If applicable, complete Parts III or IV based on your employment status. Ensure to provide accurate dates and company details as required.
In Part V, enter details for a local contact person who is a Singapore Citizen or Permanent Resident. Include their NRIC number and contact information.
For annual salary declaration in Part VI, report your fixed annual salary earned for the previous year by January 31st.
Finally, complete Part VII by signing and dating the declaration to confirm all information is true and correct before submitting via email.
