Begin with Step One: Student Information. Fill in your last name, first name, middle initial, date of birth, and myUH ID. Ensure all fields are completed; if a question does not apply, enter 'N/A' or '$0'.
Proceed to Step Two: Dependency Status. Answer questions regarding your marital status and whether you have dependents. This section determines if parental information is required.
In Step Three: Parent Information, provide details about your legal parents if necessary. If you answered 'Yes' in Step Two, skip this step.
Move to Step Four: Income Information. Report earnings for yourself and your parents as applicable. Include any untaxed income and ensure accuracy by referencing tax documents.
Complete Step Five: Household Information by listing all household members and their relationship to you.
Finally, sign the application in Step Six. Ensure that all information is accurate before submitting.
