Begin by filling in your CURRENT NAME. Ensure you provide your surname and given names clearly in BLOCK letters.
If applicable, enter any PREVIOUS/OTHER NAMES. Indicate the type of name (Maiden, Previous, Alias) and complete the fields as required.
Provide your BIRTH DETAILS including your date of birth, gender, and place of birth. Make sure all information is accurate.
Fill out your ADDRESSES section with both your current residential address and postal address if different. Note that international addresses cannot be used for posting certificates.

Answer the NATIONAL POLICE CHECK DETAILS questions regarding contact with children and vulnerable groups accurately.
