2. Begin by filling out the Requestor Information section. Print or type your name, daytime phone number, business name (if applicable), mailing address, email, and fax number.
3. Select how you would like the driving record(s) sent by choosing one of the options: Email, Fax, or U.S. mail (note that only one record can be mailed).
4. In the Drive Record Requested section, print or type the name of the individual whose record you are requesting along with their date of birth and Washington driver license number.
5. Choose the type of record requested. Ensure you include $13 for each additional record type selected.
