Enter the patient date in MM/DD/YY format at the top of the form.
Input the total number of licensed skilled nursing beds for your facility.
Fill in your CDPH license number, followed by the facility name and address.
Provide the names of both the administrator and the Director of Nursing or their designee.
Record the beginning patient census at midnight, along with scheduled direct care service hours. Calculate and enter the scheduled DHPPD by dividing total hours by beginning census.
Document any daily census changes at specified intervals (12:00am, 8:00am, and 4:00pm) including admissions, discharges, and other relevant events.
At day’s end, calculate average patient census and actual direct care service hours. Enter these figures to determine final DHPPD values.
Finally, ensure that the Director of Nursing or designee reviews and signs off on the form to confirm accuracy.
