2. In Section 1, have an FHSAA-recognized wrestling assessor fill in the school name, city, student’s name, ID, height, weight, body fat percentage, and minimum weight. Ensure all fields are accurately completed.
3. The assessor must also provide their printed name, daytime phone number, grade of the student-athlete, and date of assessment. An email address is optional but recommended for follow-up.
4. Next, Section 2 requires a physician's input. They will indicate whether it is safe for the student-athlete to wrestle during the specified school year by checking the appropriate box.
5. The physician must then provide their name, stamp, signature, and date to validate the assessment. Make sure this section is fully completed before submission.
6. Once filled out completely, give the original form to your school’s athletic director and attach a copy to the NWCA weigh-in sheet. Remember not to send it to the FHSAA office.
