1. Click ‘Get Form’ to open the prepass application in the editor.
2. Begin by completing the Account Application. Ensure all fields are filled out legibly, including your account name and USDOT number.
3. Attach copies of required operating credentials based on the states your vehicles will travel in. This includes vehicle registration cards, IFTA licenses, and insurance cards.
4. If applying for PrePass Plus or Tolls Only service, refer to the Vehicle Reference Chart included in the application. Write down the Vehicle Reference Number (VRN) on each vehicle’s registration card.
5. For PrePass Ag service applications, have an authorized representative complete the enclosed PrePass Ag Application and mail it to the specified address.
6. Submit your completed paperwork via fax, email, or mail as indicated at the top of your Account Application.
