The new Impella CP (Cardiac Power) (Fig. 2) is a 14F pump mounted on a 9F catheter and can provide peak blood flows of approximately 4 L/min using the same console platform as the Impella 2.5.
The Impella device is a catheter-based miniaturized ventricular assist device that pumps blood from left ventricle (LV) into ascending aorta and responsible for systemic circulation at an upper rate between 2.5 and 5.0 L/min.
The LV waveform is derived from the aortic placement signal and the motor current. The optical sensor on the Impella heart pump senses aortic pressure (AoP). The Impella motor current recognizes pressure gradients between the aorta and left ventricle during the cardiac cycle. ( P(AO-LV)).
The Impella heart pumps require a specific purge pressure range (300-1100 mm Hg) for optimal pump flow and a specific systemic ACT anticoagulation range (160-180 seconds) for optimal and sustained function. Studies report aPTT targets ranging between 55 and 80 seconds with optimal outcomes or anti-Xa ranges of 0.3-0.5.
