Begin by entering your IRP Account Number and Fleet Number at the top of the form. These identifiers are crucial for processing your request.
Fill in the Owner/Registrant Name and Current Mailing Address fields accurately to ensure that your new cab card is sent to the correct location.
Provide details about your vehicle, including Vehicle Identification Number, Year, Make, Body Type, Title Number, Plate Number, Unit Number, and Weight.
Select a Replacement Reason by checking the appropriate box for Damaged, Defaced, Lost, Stolen (with or without case number), or Lost-in-transit.
If applicable, enter the US DOT number assigned to your vehicle and Taxpayer Identification Number (EIN or SSN).
Indicate whether the designated carrier responsible for safety will change during the year by selecting YES or NO.
