4. List the serial numbers of the enclosed Prize Bonds in the designated area. Ensure you keep a note of these numbers for your records.
5. All bondholders must sign in the signature section provided. If any bondholder is under 18 years old, both parents/guardians must sign as well.
6. If applicable, complete Panel A for lost bonds or Panel B for a change of address. Ensure that all necessary proofs are included.
7. Finally, review all entries for accuracy before submitting your completed application form along with any required documents.
