4. Provide your permanent address in full, then enter your current address if it differs from the permanent one.
5. List your educational qualifications and specify your nationality.
6. Indicate the name of the training institution where you completed your nursing education.
7. Enter the date of registration along with the registration number assigned to you.
8. Select the type of renewal required (B.Sc. Nursing, General Nursing, etc.) and provide additional details such as religion and caste if applicable.
9. Complete the payment section by entering the date of remitting fee and SBI Collect reference number. Ensure to attach original registration certificates as instructed.
