2. Begin by filling out Section A with your current account information. Enter the name of an owner, partner, officer, or LLC manager/member along with the current UBI number and business name/trade name.
3. Proceed to Section B where you will specify the changes needed. For example, if you wish to cancel a trade name, clearly write it in the designated area. Remember that this does not affect corporation names.
4. If changing your mailing address, provide the complete street address, city, state, and zip code. Note that this cannot be used for changing a Corporation Registered Agent address.
5. For location address changes, include the new physical address without using a PO Box. Also, indicate the old location address for reference.
6. Update your contact information by entering any new phone numbers or email addresses as required.
7. Complete any additional fields such as closing locations or accounts and provide reasons where necessary before signing at the bottom of the form.
