Click ‘Get Form’ to open the mab 4 document in the editor.
Begin by entering your Federal Identification Number and the City/Town Name in the designated fields at the top of the form.
In section 1a, input your Gross Receipts from the sale of meals, including food and non-alcoholic beverages. Ensure accuracy as this is crucial for tax calculations.
Next, fill in section 1b with your Gross Receipts from the sale of alcoholic beverages. This will help you determine your total gross receipts.
Calculate your Total Gross Receipts by adding lines 1a and 1b together. Enter this amount in line 1.
If applicable, enter the Total Charged for Tax-Exempt Meals in line 2. This amount will be subtracted from your total gross receipts.
Complete line 3 by calculating your Total Taxable Receipts (line 1 minus line 2).
Proceed to calculate State Tax Due (line 3 x .0625) and Local Tax Due (line 3 x .055) in lines 4a and 4b respectively.
Finally, review all entries for accuracy, sign and date the form before submitting it as instructed.
