Calculate your Total Gross Receipts by adding lines 1a and 1b together. Enter this amount in line 1.
If applicable, enter the Total Charged for Tax-Exempt Meals in line 2. This amount will be subtracted from your total gross receipts.
Complete line 3 by calculating your Total Taxable Receipts (line 1 minus line 2).
Proceed to calculate State Tax Due (line 3 x .0625) and Local Tax Due (line 3 x .055) in lines 4a and 4b respectively.
Finally, review all entries for accuracy, sign and date the form before submitting it as instructed.
