4. Initial next to your name and LJAA Representative's name upon pickup of unsold artworks. Ensure you specify dates for artwork removal and exhibition duration.
5. Finally, sign and date the form at the bottom to confirm your agreement with LJAA. If needed, use additional space on the backside for more artworks.
