Begin by filling out the Applicant Information section. Enter your Social Insurance Number, last name, first name(s), birth date, age, and sex. Ensure all details are printed clearly.
If applicable, complete the Spouse or Partner Information section with similar details as above.
In the Consent For Release of Information section, select either Option 1 (Consent Granted) or Option 2 (Consent Not Granted) based on your preference for sharing tax information.
Proceed to Residency Information. Confirm if you have lived in B.C. for the past twelve months and provide previous addresses if necessary.
Fill out Household Information by checking applicable options and listing any other persons living with you.
Complete Contact Information with your phone numbers and email address for communication purposes.
In Rent Information, indicate whether you rent or own your home and provide details about your rent amount and conditions.
Provide Income Information by listing all sources of income and attaching necessary documentation as specified in the checklist.
Finally, review your application for completeness, sign it, attach supporting documents, and submit it to the designated address.
