The testimonial form should contain all necessary information, i.e. details of the yacht, legible signatorys email address, breakdown of service time (days at sea, standby, shipyard, and watchkeeping, where applicable)
Proof of ownership of the boat youre claiming must accompany your application. If your time was spent on friends or family members boats, you must provide a sea service form signed by the owner of the boat you intend to claim time on.
