2. In Part A, fill in your school name, student name, grade, and student number. Ensure that a parent or guardian completes this section if you are under 18.
3. Review the eligible activities checklist by scanning the QR code or visiting the provided link to confirm your activity is acceptable.
4. Once your activity is completed, move to Part B. Enter the details of your community involvement activity including the activity name, completion date, hours worked, organization name and phone number, and supervisor's name.
5. Obtain signatures from both your supervisor and yourself. If applicable, have a parent or guardian sign as well.
6. If your activity is not listed in Part A, complete Part C by providing a description of your proposed activity for principal approval before proceeding.
7. Finally, attach any proof of community involvement such as a reference letter and submit the completed form to your Secondary School Guidance Counsellor.
