1. Click ‘Get Form’ to open the Maktab Application Form in our editor.
2. Begin by filling out the 'Details of Applicant' section. Enter the surname, forename(s), and date of birth. Select the gender by ticking the appropriate box.
3. Provide your address, city, postcode, home telephone number, and mother tongue in the designated fields.
4. Next, complete the 'Details of Parent/Guardian' section for both father and mother. Include names, addresses (if different from the applicant), telephone numbers, mobile numbers, and email addresses.
5. Fill out the 'Emergency Contacts' section by providing names, addresses, telephone numbers, and relationships for two emergency contacts.
6. In the 'Details of Islamic Education' section, enter information about your current masjid, imam/teacher’s name, duration of study, and any completed Islamic education details.
7. Complete the 'Medical Details' section by providing your GP's name and contact information. Indicate any serious illnesses or allergies if applicable.
8. Review and sign the 'Parental Approval for Medication & Emergency Treatment' section before signing as a parent/guardian at the end of the form.
