2. In Section A, enter your personal details using BLOCK LETTERS. Start with your IRD number, followed by your name, postal address, and contact numbers. Ensure all fields are filled accurately.
3. Provide your bank account details for any potential refunds. This includes the bank name, branch, account number, and suffix.
4. Indicate your wish to opt out of KiwiSaver by signing and dating the form at the bottom of Section A.
5. Move to Section B and fill in your employer’s IRD number and business name, along with your employment start date.
6. If applicable, complete Section C for late opt-out requests by providing a reason if you are submitting this after eight weeks of employment.
7. Once completed, either give this form to your employer or send it directly to Inland Revenue as instructed.
