Licenses are not transferable to new owners or to a new location. The Board of Pharmacy must approve all changes in ownership and/or location BEFORE the change occurs. A new license number will be issued.
Once the license is cancelled, you must file a new application and meet all the requirements in effect at the time of reapplication.
