- Begin by entering the Host Customer Name, Telephone, Address of Facility, Billing Account Number, Meter Number, and Application ID Number in the designated fields.
- Indicate whether the Host Customer is a Municipality or Other Governmental Entity by checking the appropriate box. If applicable, attach the DPU issued Public Entity certification form.
- Answer question A regarding eligibility for net metering service by selecting 'Yes' or 'No'. If 'Yes', note that you are not eligible for net metering service.
- If applying as an Agricultural Net Metering Facility, respond to questions B1 through B4 accordingly to confirm eligibility.
- For neighborhood net metering applications, answer questions C1 through C3 to ensure all participants meet requirements.
- Complete section D on how electricity generation will be reported to the Company and select your preferred method.
- In section E, indicate how Net Metering Credits will be applied based on your preferences.
- If applicable, complete sections F and G regarding Class III Net Metering Facilities and allocation percentages.
- Finally, sign and date the application at the bottom of the form to certify its accuracy before submitting it.
