2. Begin by entering the Host Customer Name, Telephone, Address of Facility, Billing Account Number, Meter Number, and Application ID Number in the designated fields.
3. Indicate whether the Host Customer is a Municipality or Other Governmental Entity by checking the appropriate box. If applicable, attach the DPU issued Public Entity certification form.
4. Answer question A regarding eligibility for net metering service by selecting 'Yes' or 'No'. If 'Yes', note that you are not eligible for net metering service.
5. If applying as an Agricultural Net Metering Facility, respond to questions B1 through B4 accordingly to confirm eligibility.
6. For neighborhood net metering applications, answer questions C1 through C3 to ensure all participants meet requirements.
7. Complete section D on how electricity generation will be reported to the Company and select your preferred method.
8. In section E, indicate how Net Metering Credits will be applied based on your preferences.
9. If applicable, complete sections F and G regarding Class III Net Metering Facilities and allocation percentages.
10. Finally, sign and date the application at the bottom of the form to certify its accuracy before submitting it.
