A Tennessee Department of Health Certificate of Immunizations is required for all Kindergarteners, 7th graders and new students registering for school. Please Note: Schedule your childs checkup now and avoid the rush. Kindergarten immunizations can be given any time after your child turns 4 years old.
Tennessees list of required immunizations is created through the normal, open rule making process. Measles, Mumps, Rubella (2 doses of each, normally given together as MMR): if born on or after January 1, 1957 only. Varicella (2 doses or credible history of disease): if born on or after January 1, 1980 only.
Requirement: Documentation of one dose of meningococcal vaccine given on or after the sixteenth birthday. Suppose the initial vaccine for meningitis was given before the age of 16. In that case, immunization requirements will not be met unless an additional booster vaccine is given after the sixteenth birthday.
To begin a TN immunization record request, please call the TDH at (615) 741-7247 and provide the following information about the person whose record is needed: Individuals full name (first, middle, and last name) Individuals maiden name and/or all previous last names. Individuals date of birth.
