2. Begin by attaching copies of your vehicle registration, vehicle safety inspection, Hawaii No Fault insurance, and driver's license. Ensure you also include the $10 processing fee.
3. In the MCCS Facility Participation section, select one option: Klipper Golf Course, K-Bay Lanes, or Officers’ Club. Fill in the name of the MCCS Manager/Supervisor/Sponsor along with their rank/title.

5. Complete the Guest Information section by entering details about your sponsored guest such as their name, date of birth, and physical characteristics.
6. Read and agree to the Statement of Understanding by signing and dating the application. Ensure all information is accurate before submission.
