In Section B, the attending doctor must complete their details. This includes their surname, practice number, and HPCSA registration number.
Section C requires a diagnosis and medicines for which authorization is requested. Ensure all relevant documentation is attached as specified.
Finally, review all sections for accuracy before signing. Once completed, submit the form via fax or email as instructed.
