1. Click ‘Get Form’ to open the cdle request in the editor.
2. In Part I, enter the Employer Information. Fill in the Owner, Partners, or Corporate Name, Employer Account Number, Trade Name, Street Address, City, State, and ZIP Code. Ensure all fields are completed accurately.
3. Proceed to Part II if there is a change of ownership or termination. Enter the Date of Termination or Change and indicate if the employer had seasonal status. Select the appropriate reason for change from the provided options.
4. If applicable, complete Part III for changes in name or address. Specify whether it’s a physical location address change or a mailing address update and provide all required new information.
5. Finally, sign and date Part IV to certify that you are authorized to make this report. Without a signature, your request cannot be processed.
