Provide documentation of your hospital’s certification. Upload a copy of the certificate or official letter that includes essential details such as the name of the certification body, date received, and expiration date.
Prepare a CEO Attestation Letter using the provided template. Ensure it is printed on hospital letterhead and signed by your CEO or administrator before uploading it.
