2. Begin by filling out Part A with the individual's details, including their Medicare number, family name, date of birth, and address. Ensure you print in BLOCK LETTERS using a black or blue pen.
3. In Part B, list only those immunisations not already recorded on the AIR. For each vaccination given, mark it with an 'X' and specify if it was administered overseas.
4. If you are unsure of the vaccine brand name, write the generic term in the 'Other (specify)' section. Complete any planned catch-up schedules for overdue vaccines by ticking the appropriate box.
5. Finally, have a recognised vaccination provider complete Part C. They must provide their Medicare provider number or AIR registration number and sign to certify that all information is accurate.
