4. Identify the trained data submitter and their stream team number. List all participants involved in the monitoring.
5. For each net set (1, 2, and 3), specify the type of net used and record habitat type along with scores for organisms collected.
6. Circle the types of organisms collected from each net set and tally them to determine your water quality rating based on provided thresholds.
7. Complete any comments regarding changes from usual readings and indicate if fish were present by marking yes or no.
