Click ‘Get Form’ to open the medimpact form in the editor.
Begin with Part 1: Member Information. Fill in your member/cardholder ID Number, which can be found on your insurance card. Ensure all required fields are completed accurately.
In Part 2: Receipt Information, gather your prescription receipts. If necessary, have your pharmacist complete this section. Remember to tape receipts to a separate page instead of stapling them.
For each medication, provide details such as Date Filled, RX Number, Quantity, and Copay. Make sure to include all required information to avoid delays in processing.
Complete Part 3 by having the pharmacy fill in their information if it’s not available on the receipt. Don’t forget to keep a copy of everything for your records.

How long do prior authorizations take to get approved?
How long does prior authorization take? If you file an urgent request, we will have a decision provided in 72 hours or less. A standard non-urgent request may take up to seven days for us to make a decision. Learn more about the review of a non-covered drug, one not on our drug list.
