2. Fill in the date, company name, department, task, supervisor, and evaluator details at the top of the form.
3. In the Hand Activity Continuum section, circle the number that best represents the hand activity level observed during the task. Choose from 0 (idle) to 10 (continuous exertion).
4. Next, provide a Borg CR-10 scoring rating by selecting a number from 0 to 10 that reflects perceived exertion.
5. Calculate and enter your average score based on your observations in the designated field.
6. Refer to the HAL TLV Scoring Graph to visualize normalized peak force against hand activity levels.
7. Finally, check the appropriate box under Results based on your score: Below Action Limit, Above Action Limit & Below TLV, or Above TLV.
