Fill out Service Information with details such as service date, work order number, work performed, labor hours, labor amount, markup, part amount, tax, and total amount due.
In Part Information, list parts removed and replaced along with total labor costs and quantities. Don't forget to include travel allowance if applicable.
Finally, ensure that both servicer/technician and customer sign and date at the bottom of the form to certify completion.
