Begin by filling out Section A, which includes your personal details. Ensure you provide your full name, address, and contact information in BLOCK CAPITALS.
In Section B, indicate your examination details. Enter your candidate ID and select the level of licence you are applying for (Full, Intermediate, or Foundation).
If applying for a Full (Club) Licence, complete Section C by providing details of two club officials who hold a full licence.
In Section D, confirm the type of licence you are applying for and list any preferred call signs if applicable.
Complete Sections E and F as necessary, providing additional information for Temporary Reciprocal Licences and confirming consent for publication in a call sign book.
Finally, review all sections for accuracy before signing in Section H and submitting your application.
