1. Click ‘Get Form’ to open the affiliate ID card in the editor.
2. Begin by filling out the Affiliate Information section. Enter your legal name, date of birth, address, city/state, zip code, and contact information including home/cell phone and personal email. Indicate if you have been affiliated with USF before and provide any necessary details.
3. Next, complete the Department Information section. This includes entering the start date, sponsoring office/department name, supervisor's name and title, their signature, email, end date, department FOP, supervisor CWID, and date.
4. Submit the completed form to Human Resources for approval via email or in person. Ensure HR approval is documented with signatures and dates.
5. Once approved by HR, present your government-issued photo ID at the One Card Office to receive your USF One Card. Make copies of the form as needed for verification.
6. Optionally request additional university services through your sponsoring department after receiving your USF One Card.
