Unlike ROS, which asks the patient if there are symptoms in individual body systems, HPI captures the nature of the presenting problem and symptoms. The level of HPI is determined by the number of elements of HPI recorded. There are a total of 8 elements that may be included.
It includes the following elements: location , quality , severity, duration, timing, context , modifying factors, and associated signs and symptoms. Brief and extended HPIs are distinguished by the amount of detail needed to accurately characterize the clinical problem(s).
OLD CARTS is a mnemonic device used by providers to guide their interview of a patient while documenting a history of present illness. The letters stand for onset; location; duration; characteristic; alleviating and aggravating factors; radiation or relieving factors; timing; and severity.
The HPI should be written in prose with full sentences and be a narrative that builds an argument for the reason the patient was admitted. ​ Has a starting point (i.e. the patient was in her usual state of health until 5 days prior to admission.). ​ Has appropriate flow, continuity, sequence, and chronologic order.
