OLD CARTS is a mnemonic device used by providers to guide their interview of a patient while documenting a history of present illness. The letters stand for onset; location; duration; characteristic; alleviating and aggravating factors; radiation or relieving factors; timing; and severity.
Some examples include: three weeks, since childhood, diagnosed last year, or since last visit. Timing Timing refers to when a sign, symptom, or condition happens. Some examples include: daily, during the day, only at night, continuous, or intermittent.
The HPI should be written in prose with full sentences and be a narrative that builds an argument for the reason the patient was admitted. Has a starting point (i.e. the patient was in her usual state of health until 5 days prior to admission.). Has appropriate flow, continuity, sequence, and chronologic order.
