- Provide a copy of the preliminary Institutional Self-Study and select its submission format: Hard Copy, CD-ROM, or Electronic Upload.
- Enter contact information for the owner or designee who will act as liaison during the accreditation process, including name, title, phone number, and email address.
- If interested in a Saturday visit, indicate your preference and include the additional fee of $1,225.00 if applicable.
- Finally, ensure to sign and date the form before submitting it for processing.
