1. Click ‘Get Form’ to open the PA SLAM Wrestling Club Registration Form in the editor.
2. Begin by filling out the Wrestler’s Information section. Enter the wrestler's name, nickname, date of birth, age, current grade, school, coach’s name, cell number, email address, current weight (not weight class), t-shirt size, and shorts size.
3. Next, complete the Custodial Parent/Guardian Information section. Provide the names of the guardians, their address including city and zip code, home phone number, cell phone number, and email address.
4. In the Emergency Contact Info section, enter the name and phone numbers of an emergency contact.
5. Review and sign the Liability Waiver. Ensure that you understand the risks involved and provide your printed name and signature along with the date.
6. Complete the Photo/Video Release by providing your printed name and signature along with the date to grant permission for promotional use of images.
7. Finally, if applicable for club use only, fill in registration details such as date registered, payment date, amount paid, and check number.
