How to Order Requirements. Copy of DD-214 or Certification of Military Service (NA FORM 13038), discharge papers or NY State driver license displaying veteran status. Online. You must have a valid New York State registration that does not expire within 60 days. By Mail.
Veterans are eligible for one (1) set of plates for each vehicle registered in his or her name.
Complete and submit the Military Service Notification (pdf) (MV-75) to notify us that you are on active military duty. We will then automatically extend your Driver License for a maximum of six months after your separation from military service. You can then renew your license within the six months.
Disabled Veteran plates are NOT the same as Handicapped plates. I have had my DV plates since 1991. DV plates do not give you the right to park in a Handicapped parking spot.
