(a) Complete a Hardship Application, (DMV Form 735-6044). The form is available as an online application at DMV2U.oregon.gov, as a print-only application from, .oregondmv.com or at any DMV office. (b) Pay the hardship permit fee and the reinstatement fee. (d) Provide proof of identity and date of birth.
A hardship license only allows you to drive for essential purposes. Driving to a gym, a friends house, or any recreational activity is not allowed under Florida Statutes 322.271.
Some states require proof of employment, enrollment in school, or a medical condition before approving your application.
