The best way to apply for a passport and get it as quickly as possible is to apply for expedited service. You can apply for expedited service by filling out the DS-11 application form and ticking the expedited service box.
The applicant should present the following items: Completed Form DS-11. Please do not sign the document until instructed to do so by the Consul. Completed Form DS-64 providing circumstances of the loss/theft/damage.
