2. On Page A-1, fill in your personal details including your name, contact information, and current employer. Ensure you read the acknowledgment section before signing and dating the application.
3. Proceed to Page A-2. In Section A, document your Information Security Management Experience by listing each employer, job title, and duration of experience. Remember that you need at least three years in this area.
4. In Section B, provide details of your General Information Security Experience. Make sure not to duplicate any entries from Section A.
5. For Section C, indicate any substitutions for general experience such as certifications or degrees. Attach necessary documentation like transcripts if applicable.
6. Complete the Summary of Work Experience section by totaling your years from Sections A, B, and C. Ensure it meets the minimum requirement of five years.
7. Fill out the Verification page (V-1 and V-2) with your verifier's details and ensure they sign off on your work experience.
8. Collect all supporting documents and submit your completed application via fax or email to ISACA for processing.
