1. Click ‘Get Form’ to open the cost proposal template in the editor.
2. Begin by selecting your role as either Prime Consultant, Subconsultant, or 2nd Tier Subconsultant. Mark the appropriate checkbox.
3. Fill in the Consultant name, Project No., Contract No., and Date at the top of the form.
4. In the Direct Labor section, list each classification/title along with the corresponding name, hours worked, actual hourly rate, and total cost for each position.
5. Calculate and enter subtotal direct labor costs, anticipated salary increases, and total direct labor costs in their respective fields.
6. Proceed to Indirect Costs. Enter fringe benefits and overhead rates, then calculate total indirect costs based on provided formulas.
7. Complete the Fixed Fee section by entering the fixed fee percentage and calculating total fixed fee.
8. Itemize any other direct costs (ODC) and subconsultants’ costs as necessary. Ensure all calculations are accurate before finalizing.
9. Finally, certify your direct costs by filling out your name, title, signature, date of certification, email, phone number, and address at the bottom of the form.
