How to use or fill out gpcl form with our platform

- Begin by selecting the scheme you are applying for: either 'Childcare Leave' or 'Extended Childcare Leave'.
- In Part 1, fill in your personal details including your name, NRIC/FIN number, designation, and date of joining the company.
- Proceed to Part 2 and provide details about your youngest child, including their name, birth certificate number, and date of birth.
- In Part 3, declare your child's Singapore citizenship status by selecting 'Yes' or 'No'. If applicable, attach a copy of the citizenship certificate.
- For Part 4, indicate the relevant period for taking leave and specify the dates you will be on leave in the provided sections.
- Complete Part 5 by declaring any previous childcare leave taken with past employers during the current calendar year.
- Finally, review and sign in Part 6 to confirm that all information is accurate before submitting.
