1. Click ‘Get Form’ to open the Prize Money Claim Application Form (PB-23) in the editor.
2. Begin by filling in the date at the top of the form. Ensure it is accurate as it reflects when you are submitting your claim.
3. In the CHECK LIST section, mark the boxes next to each required document: Original and Copy of Prize Bond, Photocopy of valid CNIC/SNIC, and ensure two signatures on both sides of the original and copy of the Prize Bond.
4. Provide your personal information in the CLAIMANT INFORMATION section. Fill in your name as per CNIC, CNIC number, address, contact number, taxpayer status (Filer/Non-Filer), and email ID.
5. Detail your Prize Bond information by entering the series, serial number, date of issue, place of issue, draw date, and amount of prize money in the PRIZE BOND DETAILS section.
6. Select your preferred mode of payment for prize money under MODE OF PAYMENT OF PRIZE MONEY. Choose between cash or bank credit based on your total prize money.
7. Fill out your BANK ACCOUNT DETAILS if opting for bank credit. Include bank name, branch name, account title, and IBAN.
8. Read and accept the UNDER TAKING statement by signing where indicated. This confirms that all provided information is true and complete.
